Step 1 – PASS

Our program participates with ADEA PASS.  Applicants must submit a completed application by December 1 of the year before the residency program is to begin.  This will allow time to conduct interviews prior to Match deadlines. Click for more information on ADEA PASS.

            Required in application:

  • Essay
  • Dental School Transcript
  • Curriculum Vitae (CV)
  • Board results, including clinical competency
  • 2 Professional Evaluation Forms

 

Step 2 – Additional Application Materials Required

In addition to PASS application submission, applicants are required to forward the following directly to our program

  • 2”x2” photo (digital or hard copy)

Step 3 – The Match Program

The program participates in the Postdoctoral Dental Matching Program.  In order to be considered for this program, candidates must register with the Match via the PASS website during the application process. Click here for more information on the Match Program. 

 Any remaining openings after Match day will be filled with post Match candidate applicants.  Post Match candidates should send application materials directly to the program.

PCHC's twelve-month residency is specifically focused on increasing the clinical knowledge, skills, and experience of new family nurse practitioners, providing them with a positive transitional experience from school to practice, and an opportunity to fine-tune skills under the guidance of an experienced mentor.
